MyMediCard Details
The picture below is a sample of MyMediCard. Each card is specifically tailored to an individual. The individual's name, date of birth, pharmacy information (telephone number and address), the individual's telephone numbers, the Primary Care Doctors name, address and telephone numbers, and more importantly, the medications being taken, the volume, and dosage information.
MyMediCard also lists allergies and an Emergency Contact name and telephone number.
Four identical MyMediCard's with the individual's name are shipped. These cards easily fit into a pocket, wallet, pocket book, can be hung on the refrigerator, or given to a close friend or relative. Since the MyMediCard contains information that could save your life, you may want to purchase additional cards.
Four (4) additional duplicate MyMediCard's can be purchased at any time for only $7.95. Additional cards are also laminated. Please include $1.50 for shipping and handling charges per order. |
